Patient portal As part of HIXNY, Adirondack Health uses a patient portal to give patients access online to their medical records. The patient portal facilitates better communication with your physician's office by providing secure and convenient access from the comfort and privacy of your own home or office.
Patient Portal Patient Portal HIXNY is the ‘Health Information Exchange for New York State’. When visiting Urgent Care–Adirondack, our patient access team members will review with you and ask you to sign consent regarding HIXNY.

From the login screen, click on the "Start Now!" button and follow the prompts.
You will need your username and password information to access the system.
After successfully logging in, the first screen you will see is the Home page. First time users should follow the prompts to complete the login process. Each time you login to the Patient Portal you can see notifications about Appointments and New Messages.
After each office visit, a report should be posted to this portal for you to view. Select the time range and then click the "Refresh" button. You can also select a time range from "this month", "last month", "this year", "last year" or "all time".
You are expected to see your lab results, the basic health history we've recorded in our office, and a Clinical Document Architecture (CDA) under "Health Record" session. Remember, this information includes electronic data only. Service information rendered prior to the electronic data will not be entered in to the system.
