YRMC CareConnect is powered by Cerner Health, an Electronic Medical Record (EMR) that enables you to securely collect, manage and share your health information. Cerner Health has been developing electronic medical records and secure technology applications for healthcare organizations since 1979.
With YRMC CareConnect you can: Access your YRMC hospital health record from anywhere you have Internet connectivity; Review clinical summary or discharge documents from a YRMC visit; Print elements of your YRMC hospital health record to share with your physician or care team; Review laboratory results, immunization records, and your medication history
Fill out the Medical Information Release(MIR) form and secure an approval for release directly from your attending physician and the Medical Director. MIR forms are also available at the Information and Concierge. 2. Submit the approved MIR form to the Medical Records Management Department (MRMD) for processing.
You or your authorized representative can request access to or copies of your health record by completing the Authorization for the Release of Health Records form and submitting it in person, by fax, or by mail to the Health Record Department at the location where you received care.
