What is Yourtown health?

What is the patient portal and how do I use it?

TeleHealth Services. Whether in our offices, online or over the phone: YourTown Health connects you to quality healthcare for your better health. YourTown Health is proud to offer TeleHealth solutions to new and existing patients. TeleHealth virtual visits are a convenient way for some patients to get the quality healthcare you need.

How do I sign up for insurance at Yourtown health?

Patient Portal - Hometown Health Center 1-866-364-1366 Patient Portal Thank you for logging onto the Patient Portal. This portal is a tool to keep you connected with your Care Team through a convenient, safe and secure environment. If you use the portal you have access to your records 24 hours a day, seven days a week.

What is Yourtown Health?

Through a unique program available to Community Health Centers, YourTown Health is able to buy thousands of medications at drastically reduced prices. We have our own in-house pharmacy so we can pass these savings on to you. Next time you get a prescription at one of our centers, tell your provider you want it filled at the Pharmacy of Palmetto Health Council. This program is great for those who don’t have insurance, or for those who hit the Medicare “donut hole” each year and must pay full price for their medications. We can offer this only to patients at our Community Medical Centers, so if you’re having a hard time affording your medications, call your nearest Community Medical Center to set up an appointment.

Where can I get a Pap smear?

Should you need more extensive gynecological testing or treatment, you can get it at Community Medical Center of Palmetto.

What is primary care?

Part of primary care is also preventing diseases from occurring, so we provide a full range of services designed to keep you healthy. These services include cancer screenings, immunizations, annual physicals, and diagnostic lab work, in addition to education and support to help you maintain or regain good health.

What is AMC prenatal care?

They provide all your prenatal care during your pregnancy, deliver your baby at AMC, and take care of you after your baby is born. For women who don’t have insurance, we have a special program where, for $800, you get all your prenatal and postnatal care, any in-house ultrasounds, and all your prenatal vitamins. The fee is paid in installments over the course of your pregnancy. We also have a bilingual staff to help our Spanish-speaking mothers.

What is lab work?

Part of maintaining or improving health includes lab work, which can be general or disease specific. Each of our Community Medical Centers provides laboratory testing for monitoring chronic conditions, screening for other diseases, and diagnosing other conditions. Results of most of the labs come back in a day or so, but others we complete in-house and have results much more quickly. The labs we are able to complete on-site include urine pregnancy tests, rapid strep tests, blood glucose checks, and routine urinalysis.

What is behavioral health?

Our Behavioral Health providers are dedicated to promoting the well-being of our community by servicing individuals of any age experiencing an emotional or behavioral condition. We offer expert, evidence-based mental health care alongside a compassionate, holistic path to wellness. We are committed to helping you achieve your goals and the best possible treatment outcomes.

Does Palmetto Dental have a full time dentist?

Good oral health is a big part of overall good health, and prevention is the key. Our Palmetto location has full-time dentist s to provide you both preventative and curative treatment. Our dentist s can help you prevent dental problems through bi-annual cleanings, fluoride treatments, and education about proper dental hygiene. If you have an urgent problem, we provide a full range of treatments, including filling cavities, extracting teeth, performing root canals, and treating abscesses.

Preparing for Your TeleHealth Virtual Visit

There are a few things you can do before your virtual visit, especially if you’re using video to talk with your provider.

Troubleshooting Technology During TeleHealth Visit

These are a few common troubleshooting tips if you have issues starting your appointment or even during your appointment.

Step 1: Sign up for Health eAccess at your doctor's clinic

At your next doctor's appointment, simply fill out the Health eAccess request form and return it to the patient care representative. You may also contact your provider during regular clinic hours to begin the enrollment process.

Step 2: Set up your Health eAccess account

You will receive a welcome email from HealtheAccess@boystown.org. This email will contain a temporary username and password for your Health eAccess account. Click on the link in the email. After you have activated your account, you can enter in a new username and password that you can easily remember.

Step 3: Stay connected at Health eAccess

Anytime a message is posted to your Health eAccess account, you will be notified by an email. Add HealtheAccess@boystown.org to your safe senders list to ensure you never miss a notice.

Will all of my medical records be available online?

For patient privacy, we will not upload all of your personal health information online. If you would like access to medical records not found in your Health eAccess account, please submit a medical record request through your online account.

What if I forget my password?

If you forget your password, you may reset it through your patient portal account. On the log-in page, select “Need Help With Your Username and Password?" and follow the online prompts.

Can I access my medical records from multiple Boys Town physicians?

Yes. With one Health eAccess account you will be able to access your or your minor child's medical records throughout the Boys Town National Research Hospital health system.

What is the response time for replies to messages I send through my Health eAccess account?

During regular business hours we will make every effort to respond to your online request within 24 hours. Please allow up to two business days for your provider's office to respond. We cannot review messages received after business hours, on holidays or during weekends until the next business day.
