Licensed Clinical Social Worker 1 Providing mental health diagnostic and counseling services to patients as part of the overall medical and behavioral health team. 2 Assisting patients with locating the proper resources to address their medical and psychosocial needs. Assisting patients in addressing the social determinants that may be affecting their health. 3 Assisting patients with Advance Care Planning, including selection of healthcare proxies, durable powers of attorney for healthcare, living will completion, and life-sustaining treatment decisions. 4 Working with local hospitals to assist with patient discharge and obtaining post-ER and post-hospital care. 5 Other duties as needed for any patient with a medical, mental, or social need that a LCSW can help fill.
