Call 1-855-513-5513 and a MyChart representative can provide a new activation code. During your first session, you'll be prompted to activate with your code, create a MyChart ID, a new password and choose and answer a security question.
You may submit the form via email at yhmedicalrecords@yale.edu, by mail to Yale Health, P.O. Box 208237, New Haven, CT 06520-82327 or by fax to 203-436-5536. The cost for a copy of your medical record is a flat fee of $6.50.
In order to receive Epic access, a user must first be assigned an Epic ID and go through Epic training. To request an Epic ID, contact the Yale New Haven Health Help Desk and request an Access Service Request Form. Processing the Access Service Request form may take up to 10-15 business days.

Minor proxy access for MyChart allows a parent or guardian to securely communicate on behalf of a patient who is under the age of 18. Access is obtained by completing the Access Authorization with Minor Proxy form. The proxy access becomes more limited on the child’s 13th birthday and is automatically terminated on the child’s 18th birthday.
If you are admitted to a hospital in the Yale New Haven Health system , you will be able to view your schedule of medications and testing that take will take place throughout your admission. Once you are discharged, you will be able to view your date of admission and discharge along with your discharge instructions.
