How do I upload my health forms to the patient portal?

Online: Patient Portal. Questions for RN: (320) 363-5370. Log in with your CSB/SJU network username and password. WAITING ROOMS. CSB Health Services: Separate waiting rooms are available for Well patients and Ill patients. SJU Health Services: Well patients are scheduled in the morning. Ill patients are scheduled in the afternoon.

San Juan Regional Medical Center is pleased to bring our patients a new and improved patient portal. This patient portal offers patients the ability to be a part of their healthcare and stay better informed about their care. Patients can view their health information, patient appointments, view and pay their bill, and more.

You can schedule an appointment on the Online Patient Portal. Student Health Services operates on an appointment basis. This system decreases the time spent waiting to be seen. Due to the ongoing COVID-19, walk-ins are not accepted at this time. If you call for a same day appointment, you will be seen at the first opening in the schedule.

Immunization Documentation

All immunization documentation should be submitted via the St. John’s Student Online Patient Portal. We are no longer accepting documents via mail or email.
