Patients can get an activation code after their clinic visit or they can request an activation code by completing the online request form. This code will enable you to login and create your own username and password.

MyChart's Hello Patient feature leverages geolocation capabilities on iOS and Android devices to detect when a patient arrives for an appointment. If the patient has completed all check in tasks ahead of time, the patient can be automatically checked in without even needing to visit the front desk.

Get your medical record number (MRN) and activation code by calling the Patient Portal Help Desk at 734-615-0872 (7:30 am - 4 pm). Then create your account at MyUofMHealth.org by clicking the blue "Sign Up Now" button.

Your MyUofMHealth username must be between five and twenty characters, but cannot include symbols. Once selected, your MyUofMHealth username cannot be changed. Your MyUofMHealth password must be a minimum of eight characters and must contain at least one number and one letter.

If you have signed up for an account on the MyUofMHealth (link is external) patient portal, you can manage and request appointments with your U-M physician online. Under the “Appointments” tab on MyUofMHealth, you can view upcoming or past appointments, cancel appointments up to 24 hours before your schedule time, and request a new appointment.
