How do I create an account with SLUHN?

St. Luke's University Health Network is a non-profit, regional, fully integrated, nationally recognized network providing services at 12 hospitals sites and 300+ outpatient sites. Network’s service area includes 11 counties: Lehigh, Northampton, Berks, Bucks, Carbon, Montgomery, Monroe, Schuylkill and Luzerne counties in Pennsylvania and Warren and Hunterdon counties in New Jersey.

To make it easy for you, you can download and print our Medical Information Release form or obtain a form from any patient care unit at a St. Luke’s University Health Network facility. You can request records from St. Luke’s Medical Records Department: Mail the completed Medical Information Release form.
