How do I sign up for wholehealth Chicago's patient portal?

To improve your access to timely care and comply with privacy guidelines, WholeHealth Chicago uses a secure patient portal. Send non-urgent questions to our clinical team. For existing patient accounts login below. If you are not yet registered for our patient portal, give us a call at 773-296-6700, and we can get you signed up!

What do I need to know about the patient portal?

Your password must be at least 8 characters long and include at least one number or special character. The patient portal gives you access to your medical records and lets you securely communicate with your doctors. When you sign up, you will receive an email with instructions for logging in.

View automatic updates of Lab Results and Prescriptions from your provider Share Clinical Documents with your Care Team to discuss Progress and for Opinion.
