In order to accomplish this goal, it is important that we have your child’s previous medical records, as well as any information regarding medical care received elsewhere after becoming a patient at Raleigh Pediatrics. This includes, but is not limited to office visits, immunizations, medications, and test results.

In my free time, I make sure to find time each day to work out for my own personal health and fitness. My husband and I co-own a CrossFit gym in town, and when I am not working out or working, I find time to coach CrossFit. I am CrossFit Level 1 certified, and I have a CrossFit Kids certification.
I spend most of my hours away from work spending time with my husband, daughter, and dog. My favorite hobbies are reading for fun, yoga, and baking. I also love to travel and trying out new restaurants.
At Raleigh Group, I am most looking forward to building relationships with my patients and their families, as well as working with them to make healthy lifestyle habits that will last a lifetime.
Received BA degree from the University of Chattanooga 1967 and then an MD from the University of Tennessee 1970. After completing 3 years of pediatric residency with a program at St. Jude Children’s Hospital, served as a Major and head of the pediatric department with the US Army at Fort Rucker Alabama.
My heart is with my family of three amazing children and nine exceptional grandchildren. Keeping in contact and being of service to them is most important. Reading, listening to music, exercising and working in my yard are my day-to-day pleasures, when I am not working on crossword puzzles.
To help you request a transfer of your records to another office or clinic, we have provided a Records Release Form. The blank form may be printed and then filled out by hand, or you may fill out the form electronically while online, then print the completed form.
The following forms allow patients who are 18 years of age and older to inform Raleigh Pediatric Associates how they would like their Health Information used and with whom such information can be shared.
