Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call during regular business hours.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

Please plan to take care of co-payments, co-insurance and any outstanding balances at the time of service. Co-payments or deductibles must be paid at the time of visit. We accept debit cards, Visa, Mastercard and American Express.

Insurance

Our staff will make every effort to simplify all billing and insurance claim procedures. We participate with many insurance plans; however, we recommend you call your insurance company prior to your visit to verify coverage and to determine if a referral from your primary care physician is necessary.

Self Pay Policy

If you do not have insurance we require a $132 payment-in-full for a first time visit as a new patient. All appointments thereafter as an established patient will require an $88 payment due at the time of service. These amounts reflect a 35% discount for self-pay patients and apply to all billed services.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Refill Requests

We are happy to help with refill requests during regular office hours, so please call us at (208) 459-4667 to let us know what you need.
