What is the patient portal and how does it work?

Our Patient Portal permits you to request prescription refills, view and download documents related to your medical care, and see all future scheduled appointments. Our system will authenticate you with the following information: last four digits of your social security number, your primary telephone number, your DOB, and your email address.

How amazing is Lyerly group?

Amazing because Lyerly is generally considered an EXCELLENT GROUP. For my first visit he required me to bring an MRI...he told me it was no good. He insisted on a Myelogram ..which was over 6k and goes to his Baptist quote. He failed to do the required Informed Consent. The test was worse than having a baby.

We want to make your experience with us a positive one, and we know that parking can sometimes be a challenge, so please plan to arrive on campus and park 45 minutes before your appointment so you don’t feel rushed.

What You Will Need to Bring

If you have received our patient packet by mail, please complete all pre-registration forms and bring them with you to your appointment. In addition, please bring any related medical records, imaging films or disks that have been requested. You will need your driver’s license, insurance card (s), and co-pay.

Directions and Parking

Our main office is located on the 11th floor of the Weaver Tower on the campus of Baptist Medical Center Jacksonville. Park in the garage labeled P2.

Appointment Cancellation

Please notify us at least 24 hours in advance if you find it necessary to cancel an appointment. This gives us the opportunity to schedule another patient at that time.

Preparing for Your Visit

If you provide your insurance information, we will file your insurance claim for you. Please note that you will be responsible for any co-pay, co-insurance, deductible, or non-covered services at the time of each visit.

Deductible

Depending on the type of insurance you have, you may have an annual deductible to meet. Our office will make every effort to verify your insurance benefits prior to your visit. When we call to confirm your appointment, we will inform you of the deductible. You will be asked to pay the deductible at the time of service.

Out of Network Services

Some insurance plans allow you to go out of network to see a provider without a referral. Usually, you will have a deductible to meet and a higher percentage to pay "out of pocket.”

Referral or Authorization

Many insurance plans require that you obtain a referral from your primary care physician (PCP) before you see a specialist. Therefore, we ask that you call our office prior to your visit to confirm that your PCP has provided us with a referral.

Collections

A patient with a balance that has been placed in collections must pay any balance owed to the practice in cash before the practice will see them again.
