Once you are set up with a username and password, you can log into the patient portal using any computer, tablet or smartphone with an internet connection. Self-enrollment is also available at a later time by providing your Medical Record Number and last 4 digits of your Social Security Number, or by providing your email address on file.

This new patient portal provides 24/7 online access to your health records from any computer, tablet or smartphone. Once you have an account, access your records by logging onto www.midlandhealth.iqhealth.com or download the HealtheLife app on your iOS or Android devices.
To create an account for yourself, register online or ask your health care provider to email you an invitation during your next visit. If you register online, you can choose to receive an access code via email or text message. That code will be used to create your new account.

Minor patients ages 13-17 may register individually for access to the My Health Records Patient Portal, unless determined by their physician, that the minor patient is not competent. Absent such a determination, the minor patient will control his or her own medical information.
