Our new patient portal provides students with the ability to electronically submit health forms and immunization documents required by Massachusetts law. Important COVID-19 Information

All enrolled students are required to be fully vaccinated for COVID-19 to live, learn or visit any UMass Lowell campus or property. According to the CDC, fully vaccinated is two weeks after the second dose of a two-dose series or single-dose vaccine.

Online Health Record

All newly enrolled students should complete the health forms online and upload supporting medical documentation through the Health Services Patient Portal link above.

Due Dates

All health records are required by Aug. 1 for fall enrollment and Jan. 10 for spring enrollment.
