How old do you have to be to have access to your child's medical records?

Parents/legal guardians of children under 12 years of age can enroll for access to their child's account. Access will be automatically revoked when that child turns 12 years old in compliance with privacy regulations. You can enroll for access to your child's medical information at your next Pediatric appointment.

Can a parent access a minor's health records?

As a parent or guardian, you can use Health-e-Connect to access the health records of your minor child. Patient privacy is extremely important to us, so we have developed a policy around parents' and guardians' access to their child's patient portal that follows state and federal laws.

What is My Portal (Patient Portal)?

My Portal (Patient Portal) is a website that allows you a convenient and secure way to view portions of your medical records and communicate with your provider’s health care team. Within your My Portal (Patient Portal) account, you will be able to do the following:

How can I create an account?

At your next visit, tell the staff you would like access to My Portal (Patient Portal). The staff will request your email address, and may have you complete a consent form. Within 30 minutes, you will receive an email invitation with a link to enroll. Just click on the link and follow the instructions to complete your enrollment.

Can I create an account for a family member who has a Dignity Health doctor?

Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a My Portal (Patient Portal) account with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age.

Who has access to my account?

Some of the information in your My Portal (Patient Portal) account is provided through your medical health record and is viewable by you and authorized staff of Dignity Health facilities. If you would like someone to have access to your records to help manage your care, you can sign a release to authorize a trusted relative to be set up as a proxy.

If my email invitation expires or is lost, how can I request a new one?
