How do I join the CCHC patient portal?

The CCHC Patient Portal gives you access to medical information from the many participating organizations of CCHC. Please click here to see a complete list of participating providers and medical organizations with CCHC. Call or visit your physician’s office and request an invitation to join CCHC.

Your doctor's office will provide you with a username and password to give you Patient Portal access. You can also get access by calling our Customer Service Department at (614) 423-6916 or toll-free at (844) 245-5538. The first time you log in, you will need to review the consent forms and click on next, then check the box, and click on agree.

Yes, any doctor that is a participant of CCHC will be able to view your patient record if they attest to having your author ization to view your record. If you have an Online account, you will be able to review anyone who has accessed or viewed your record.

You will receive a temporary password from them. Your temporary password will only be used the first time you log in to your portal account. You will then be prompted to create a new password and to set up security questions.

Telehealth Visits

In an effort to prevent the spread of disease, we are working to offer virtual Telehealth visits instead of in-person visits for certain situations.
