How do I login to the patient portal?

You need to enter your User Name (we suggest you use your "100" number), your University ID (your "100" number), and your Birth Date (mm/dd/yyyy) and click Submit. If you have already registered for the Patient Portal, please enter your User Name and Password. If you have forgotten your password, click "Forgot Password".

What determines which portal I use for my medical records?

The location where you receive treatment determines which portal you use. If you receive care at multiple locations (for example, if your primary care provider is at USA Mobile Diagnostic Center but you receive specialty care at Neurology Specialists), you may have records in both portals.

What can I do with my USA health portal?

You also can view or cancel upcoming appointments, message your provider directly, see your visit history, and access medical records. If you do not have an account, talk with your USA Health provider about creating one. The location where you receive treatment determines which portal you use.
