Please Bring These Items to Your Appointment 1 Completed health history form (if available) 2 Copay (the amount you need to pay for medical care, listed on your health insurance card) 3 Identification 4 Insurance information (if applicable) 5 List of allergies 6 List of questions or concerns to discuss with your doctor 7 List of prescription and over-the-counter medications you take, including supplements, vitamins and herbs you take 8 Medical record number (if available) 9 Names, addresses and reports of providers you have seen outside UNM Health, as well as test results from those appointments 10 Paperwork for Family and Medical Leave Act or disability benefits 11 Record of your vaccinations 12 Records of your blood pressure, blood sugar and daily weight (if applicable) 13 Referral and/or authorization forms, if required by your insurance provider
