If you have had at least one appointment with Medical Services (MS) and would like to release your medical information to yourself or to another person/clinic, please complete the electronic “Authorization to Release Health Information” form through the MyCUHealth portal . Record requests will be processed within 30 days.
We recommend that any student with a chronic medical condition have their doctor's office send a copy of pertinent medical records to Wardenburg Health Center. Alternatively, students can upload any records to the MyCUHealth portal.

You have the right to request an amendment to your medical record if you believe it is incorrect or incomplete. Please submit a completed Amendment Request Form to FAX-Amendment@uchealth.com or fax the request to 513-584-5191. You may also send your request via mail to the appropriate UC Health location address listed on this page.
