Welcome to the MyUnityPoint Patient Portal The MyUnityPoint patient portal powered by MyChart is an internet application that enables a patient to have secure web-based access to selected medical information about the patient.

How do I get MyUnityPoint activation code?

You may also call your primary care clinic or our support team at (877) 224-4430 to get an activation code or ask to sign up during your next office visit.

How do I add a child to Unitypoint MyChart?

Once logged in, select “Share My Record” on the drop down menu. Select “Friends and Family Access” in the “Share with People” section. Select the applicable question for your request under the “Who Can See My Record” section.

What is proxy access medical?

Proxy access allows parents, legal guardians or caregivers to access a patient's health. information on the patient portal. Proxy access is an available option for adult patients. (18 years) and pediatric patients (newborn – 12 years of age).

Your priest, minister or rabbi is always welcome to visit you.

Chaplains are available to Trinity patients and families for supportive visits and pastoral care. During business hours, Monday-Friday, 8 a.m.

Are worship services available?

The Bettendorf, Moline and Rock Island campuses each have a chapel/meditation room that is open to visitors of all faiths for prayer and meditation. A Roman Catholic Mass is celebrated each Sunday in the Rock Island chapel at 9:30 a.m. and patients may participate from their rooms via Simulcast on channel 9 of the television.

Spiritual volunteers

Befrienders at Trinity are community volunteers who are members of a faith community and receive training through the Pastoral Care department to visits hospitalized patients. Those interested in becoming a Befriender may contact Pastoral Care by calling (309) 779-2989.
