UHealthe and your health record The UHealthepatient portal is a secure website provided to patients of Uniontown Hospital to allow them access to parts of their healthrecord. If your appointment was through Union Hospital’s outpatient physician network (Union Physician Services), you will have access to your electronic medical record through Cleveland Clinic My Chart. If you were admitted to Union Hospital, please use Union Hospital’s Patient Portal to gain access to your electronic medical record.

Only you and/or a person you have legally authorized to can access your Patient Portal. A Patient Portal proxy form is available to allow parents or legal guardians access to a patient's online medical record.
Minor: Parents may have access to minor patient records (0-17) by contacting the Medical Records department for Patient Portal Proxy form and present a copy of ID.
Call Health Information Management at 330.343.3311, ext. 2427. Please call during regular business hours. You will be asked for your email address and other identifying information. Once your ID is confirmed, you will receive an email message with instructions to set up your Portal account.
All of the information in the Patient Portal comes from your Union Hospital Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.
There is a Contact Us section on the home page of the Patient Portal that allows you to ask questions about the operation of the Patient Portal.
The Union Hospital Patient Portal is available through a free mobile app. Compatible with iOS 10+ or Android 5+ on both smartphone or tablet. From your mobile device, simply search "Mhealth" or "Meditech Mhealth" at the Google Play or Apple App Store or tap the appropriate icon below. Once you install the app, simply sign in with your Patient Portal Logon ID and Password to access your information.
For parents or caregivers who need proxy access to a patient’s MyWVUChart account (including children up to age 18), please inquire at your primary care provider’s office or make the request via your MyWVUChart account by clicking “Request Family Access” under “Messaging.”

We afford the same degree of confidentiality to medical information stored on MyWVUChart as is given to medical information stored by WVU Medicine in any other medium. Firewall, passwords, encryption, and secure activation codes are used to safeguard your information. Unlike conventional email, all MyWVUChart messaging is done while you are securely logged on to the system.
Patients who are users of MyWVUChart should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyWVUChart. This means that any person with access to a patient’s e-mail will be able to see this notification. This could include the patient’s spouse, employer, or anyone else that can access a patient’s e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyWVUChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.
