UK HealthCare launched a new patient portal – UK HealthCare MyChart – on June 5, 2021. MyChart has replaced our former patient portal accessed through the FollowMyHealth platform. We encourage you to set up a MyChart account. With a MyChart account, you can continue to access information in your UK HealthCare record.
Oct 15, 2021 · Dear Patients, As you may already know, Dr. Katrina Maria Crader passed away on June 4, 2021. It has been a great pleasure for Dr. Crader and the staff of UK Psych to have provided your mental health care needs over the years, and it is not easy for our family to give up this practice legacy on behalf of Dr. Crader.
If you are registered with a GP surgery in England and you are referred by a GP to a consultant or specialist in mental health, you have the legal right to choose the organisation (qualified provider) to whom you are referred, as long as that organisation is providing that service in another part of England.
A short covering letter stating that you wish to refer this patient to Psychiatry-UK, for an ADHD assessment, under NHS right to choose legislation. Please give a brief explanation for referral. This letter should be addressed to Psychiatry-UK and should have the GP’s name at the bottom (not ANP or other).
