‘My Health and Care Record’ is a partnership between this Trust and a company called Patients Know Best (PKB). PKB work with a wide range of trusts including Western Sussex Healthcare NHS Trust and Brighton and Sussex University Hospitals NHS Trust.
The After Trauma team will be using My Health Record to follow up with some patients following recent treatment at either Conquest Hospital or Eastbourne District General Hospital. My Health and Care Record will be used to provide patients with information to support their recovery and they are also being asked to complete a questionnaire to see how they are getting on and identify if any additional support is required. Patients will be contacted through a letter, inviting them to register. If patients choose not to register then the follow up questionnaire will be sent through the post. EpicCare Link is the preferred method for submitting referrals to UK HealthCare. Alternatively, you can call UKMDs at 800-888-5533 and indicate that you would like to place a referral to a specific specialty. UKMDs will then connect you with the appropriate area for processing your referral.
There are nine user roles within EpicCare Link. Each user role has a set of identified features and permissions within EpicCare Link. For patients age 12 through 17 limited proxy access allows a parent or guardian to: 1 Pay bills online. 2 Send a secure email message to the child’s care team, as long as the patient had an established relationship with that provider before turning 12. 3 View and print past results, radiology reports and office visit summaries. 4 View and print past immunization records. 5 View and print past provider’s notes from clinic visits and discharge summaries for hospital stays. 6 View and print past discharge instructions. 7 View documentation on sports physicals. 8 E-sign UK TeleCare documents.
A parent or guardian has “full” access to their child’s UK HealthCare record until the child turns 12 and “limited” access until the child turns 18. Access for a “pediatric proxy” is automatically transitioned to “limited” access for the parent or guardian on the child’s 12th birthday. You may direct questions about proxy access to your child’s healthcare provider or to the Health Information Management office at UK HealthCare, 859-323-5117.
Note that proxy access is granted strictly at the discretion of UK HealthCare and may be revoked at any time. You must make a separate request for each patient for whom you are requesting proxy access.
