Records of deceased patients may be released to the designated Personal Representative/Successor Personal Representative as stated in a written will or by the Probate court as either the Executor or Administrator of the deceased person’s estate; the beneficiary of the patient’s life insurance policy; and the Heir at Law (a person who is legally entitled by state law to inherit property of a deceased person when that person dies without a valid will). In addition to Authorization to Release Patient Information form, the Heir at Law should complete the Affidavit of Heir form below.
At the University of Michigan, we participate in a state-wide health information exchange (HIE) that allows other doctors and hospitals outside of U of M to access information that we’ve collected about patients. Also, there are other ways that patient information is shared electronically with health care providers outside of U of M. Requests for medical records of deceased patients require a letter of authority in addition to your signed request. The letter of authority is given to the executor of a person's estate by the Probate Court upon their death. Releasing records to anyone other than the executor is illegal, as stated in Michigan Court Law 600.2157. Please also include your phone number in case we need to contact you for additional information concerning your request.
The patient's signature or a patient's legal representative's signature. Authorizations signed by a representative must contain a copy of the guardianship papers or power of attorney. Date of the signature.
Records can be released to anyone that the patient authorizes (in writing). A valid authorization MUST contain the following information or the request will be returned:
Your medical record number must be entered using 9 digits. If your medical record number is less than 9 digits, add zeros to the beginning of your number. Example: 123456 must be entered as 000123456

You must be the child's parent or legal guardian to access the record. Once a child turns 18, access is automatically revoked. Due to UMHCC Policy, there are limitations based on a patient's age restricing what you can view in the your child's/ward's MyUofMHealth account:

Call UHS Radiology directly (734-764-8302) to request radiology images, minimize wait time and ask any questions. Images will be put on a CD to be picked up or mailed.
Due to Federal confidentiality law, UHS is unable to communicate with parents without patient consent. This law protects patient privacy and controls how much health information is access and disclosed.
Please note that UHS retains records for 10 years past last date of service.
It is helpful to have your health records transferred to UHS so your health care provider knows exactly what medication or treatment was prescribed.
If you are 18 years old or older, or an emancipated minor (defined under Michigan law as being married, emancipated through a court order, or on active duty in the armed forces prior to age 18), your health records will be released only with your written consent unless permitted by law.
The Michigan Medicine Notice of Privacy Practices describes how health information about you may be used and disclosed and how you may access this information.
To enroll yourself in the "My Health" patient portal, you'll need your medical records number (MRN).
