New Medical Patients. For your convenience, our new patient paperwork for medical appointments is available to complete 100% online with a smartphone, tablet, or computer before your appointment. You can even begin filling out your forms, save your progress, and come back to finish them when you have time. ADULT New Patient Medical Intake Packet.
Our patient portals offer you convenient, personalized, and secure online access to part of your health records from participating Munson Healthcare hospitals, clinics, and doctor’s offices: 24 hours a day, 7 days a week. Below is a full list of patient portals practices currently connected within Munson Healthcare.
Patient Portal Changes. With our computer system changes, we will no longer be using the Follow My Health patient portal. Though you will still be able to access your FMH patient portal account through the direct apps, there will be no updates to your portal information after January 15, 2018.

We’re happy to announce that LTPC is now able to offer Telehealth visits. Patients must have a valid email address on file with LTPC in order to qualify for a telehealth visit.
