Morning monitoring call backs are typically done by 3:00-4:00 pm. Call your office if you have not received your text/email or a call by that time.
When possible, review your portal instructions before 3:00 pm so that you may speak with a nurse, while we are still in the office, if you have questions about your instructions. Questions regarding information viewed on the Portal should be directed to your team during regular office hours (Monday-Friday 8:30 am-4:30 pm).
To obtain your own medical records: Print out the Consent to Release of Information form, and complete as many areas as you are able. Bring this completed form to the medical records department, and you can pick up your records.
To obtain records for someone other than yourself: The Consent to Release of Information form must be completed and signed by the patient. The person picking up the records should be identified in the area called "Information to Be Released to" in the upper left corner.
If the person is unable to sign, the reason should be indicated and the legal representative of the patient will need to sign, with the relationship indicated. For example: An adult child who has a durable healthcare power of attorney would sign and indicate "DPOA" in the area titled "Relationship to Patient.".
The executor or administrator of a decedent's estate may obtain records if they provide a letter testamentary, a letter of administration or a "short certificate" showing that they are authorized to act as the personal representative of the decedent.
To obtain records for a minor: Regardless of custodial arrangements, either parent may sign the authorization to obtain records of their child. The only exceptions are if: (1) a court issues an order limiting a parent's right to review a child's medical records; or (2) a parent has given up all parental rights.
