The Silent Patient is a pretty balanced book, as you can see by the CAWPILE rating. The middle section of the book drags a bit because it’s mainly about Theo running around from character to character, asking questions about Alicia. But when it picks up again, you’re in for a ride.

The Silent Patient fails as a psychological thriller on all fronts. Its narrator lacks any credibility as a psychotherapist, as an employee, as a spouse, as a man. All characters are flat and inauthentic. The plot doesn't hang together.
The Silent Patient by Alex Michaelides, A twisty psychological thriller. In The Silent Patient, Alicia Berenson is a well-known painter who murdered her husband six years ago and hasn't spoken a word since. He was found bound to a chair with gunshot wounds to his face, and she was convicted soon thereafter.
What happens at the end of The Silent Patient. At the end of The Silent Patient, Theo goes to confront the guy that Kathy has been cheating on him with and we find out that it’s Gabriel. Gabriel’s wife is Alicia Berenson. In other words, the entire book has been told jumping back and forth in time.
The author highlights the theme of love in the novel through the marriages of the protagonist and the antagonist. Theo was deeply in love with Kathy, while Alicia had fallen in love with Gabriel. Sadly, this love is threatened by the betrayal of their partners.
The book ends with the police coming to question him about the diary. The big twist is when you realize that they are two different timelines (one before the murder, the other after) being told at once, and Theo was the “masked man” that prompted the murder (though Alicia is the one who actually did it).

Plot Summary of The Silent Patient Six years ago, a neighbor heard shots at their home and called the police. Gabriel was found tied to a chair, shot dead. Alicia was covered in blood, her wrists cut. Alicia was alone with the body and her fingerprints were on the gun. After her husband's death, she never spoke again.
Plus, it wasn't his intended consequence either. He just wanted to open Alicia's eyes - not actually kill anyone - he defintely didn't mean to cause any harm to Alicia. So, yes, he wanted to help Alicia. I also think that he was really confused by the painting and wanted to figure out the meaning behind it.
When the detective started reading the diary, he felt relieved. So the last paragraph, catching snowflakes is about him feeling free of the guilt associated with being the catalyst to Alices downfall and ultimately coma, same freedom he felt when he was a kid.

Michaelides has stated elsewhere that THE SILENT PATIENT was inspired in part by his prior part-time employment in a secure psychiatric unit. Although undoubtedly a psychological thriller, there is a strong mystery at the heart of it that begins beating at the very start of the book.
