Why joincareteam with SWLA CHS?

SWLA Center for Health Services offers its patients a prenatal class “The Baby Shower”. The class consists of lectures, discussions, and exercises, all led by a trained instructor. While approaches vary, the goal is to provide you with information to prepare for childbirth, help you make informed decisions, and minimize your fears.

SWLA Center for Health Services is a 501 (c)3 non-profit community-based Federally Qualified Health Center (FQHC) offering comprehensive medical, dental, mental health, and specialty healthcare services. SWLA is a Health Center Program grantee under 42 U.S.C. 254b, and deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

The Family Care Center of SWLA is an independent health care provider. We are affiliated with WCCH, Cornerstone Hospital, St. Pats and LCMH. We are committed to QUALITY healthcare of Westlake and surrounding communities and are here to stay. We have recently achieved a level three Virtual PCMH status and are proud to be a Quality Blue Provider.

billing

SWLA Center for Health Services will bill your primary and secondary health care plan on your behalf. However, you are ultimately responsible for the cost of services provided. You may be billed if your healthcare plan does not process in a timely manner. Further follow up with your insurance will be your responsibility.
