How do I join the West Durham patient portal?

The portal is available to current patients of The West Durham Family Health Team. DO NOT SEND YOUR EMAIL ADDRESS THROUGH THIS PAGE IF YOU’RE ALREADY REGISTERED WITH OUR PATIENT PORTAL. Patients will be sent an invitation by email to join the portal.

The Patient Portal is a free, secure online tool that gives you the ability to take charge of your health! Ability to view and print your medical records from hospital and/or clinic visits to review with your primary care provider to discuss best treatment options

How do I get access to the SW health portal?

If you did not provide an email address during a past visit, you can contact us a portal@swhealth.org and request to be registered for the portal (s). Please be sure to specify in your email whether you want access to the Hospital Portal, Clinic Portal, or both. Is there an age requirement to use the portal? Yes, for the Clinic Portal.

Southwest Durham Family Medicine, PLLC, is an independent, private medical practice. We are conveniently located near the intersection of NC Hwy 54 and NC Hwy 751 (commonly known as Hwy 54 and Hope Valley Road).

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – South Baldwin Regional Medical Center will send a text and / or email to the email address and / or cell phone number you provided upon registration to the hospital. Follow the instructions in that message to create your account.
