The UC Student Health Insurance Plan (UC SHIP) is a comprehensive medical insurance program offered to UCLA students. When a student requires care beyond CAPS’ scope of service, UC SHIP and CAPS provide medically-appropriate referrals to Behavioral Health Services (BHS), the Anthem network, and out-of-network providers.
In order to opt out of UC SHIP, students must have insurance coverage that meets or exceeds all of the Academic Plan Year Waiver Criteria. UC SHIP and UCLA have partnered with AHP (Academic Health Plans) to audit our health fee waiver requests.
Students* within their designated waiver period can submit their application to initiate the audit process through the UCSHIP Waiver Application below.
Students with approved waivers will have the UCSHIP fee credited back to their BruinBill. Credits can take up to 7 business days to appear.
Only registered students can submit waiver applications during their designated Waiver Period. Parents and families are unable to submit the waiver application using third-party access.
Students must have insurance coverage that meets or exceeds all of the Academic Plan Year Waiver Criteria. Health plans that do not meet this criteria result in a denied waiver.
UC SHIP members only pay $10 copay for the annual eye exam (with or without dilation).
The contact lens fitting and evaluation is an additional $55 to $85 on top of the $10 exam copay, depending on the contact lenses you and doctor will decide on. If you wear specialty contacts, there may be exceptions but your doctor can discuss this with you.
Good news! Your annual UC SHIP insurance benefits provide coverage towards prescription glasses and contact lenses both! Here are the details:
Optos retinal imaging (extra fee applies). Discounted extra pairs of glasses (40% for complete pair after first pair purchase). Discounted eyeglasses lens treatments which include custom sun colors, blue light protection, anti-reflective coatings and transition lenses which get dark outside. Free glasses cleanings, adjustments and repairs.

CAPS clinicians will assist students in determining the best referral on a case-by-case basis.
Referrals are valid for 12 months: Students should obtain a copy of their Anthem referral from CAPS to give to their off-campus provider. Students are responsible for renewing their referrals after 12 months and payment is not guaranteed if referrals are not renewed. Find a quick guide to benefits.
To find an in-network provider, call Anthem member services at 866-940-8306. You may also search the Anthem website. You can also get help finding a provider on the Anthem website. Some providers may request a copy of your referral, which you can obtain at CAPS.
Start calling some of the providers on the list. You may need to call more than one person if the first person you try doesn’t call you back in 24-48 hours. If you need a therapist and a psychiatrist, you will need to call both types of providers. Providers almost never pick up the phone.
If you have a mental health emergency, please call 911 or go directly to your local ER without obtaining a referral.
