Paying Your Bill

Please refer to website listed on your patient statement for payments. We do not use any third party payment sites such as www.doxo.com as these could result in additional fees to our patients.

Insurance Participation

At Sunrise Medical Laboratories, we are the preferred provider for many major HMO's, PPO's, and conventional insurance plans, such as Aetna, Oxford, United Healthcare, NYS Empire Plan, and MagnaCare, to name a few. If you don't see a particular carrier on the list, especially one you currently have coverage with, feel free to Contact Us.

Our office has recently been coming across patients with real random insurances. Actually, not even real insurance, and it says so right on the cards the patients end up receiving. People are typing in NYS insurance or marketplace and the first couple sites that pop up take you to brokers that sell you plans that are NOT INSURANCE.

Requests should be sent from your insurance company, attorney, or Disability Determination Service and mailed to the address below. We cannot accept faxes and email requests.
