What are the benefits of patient portal?

Hawthorn’s patient portal, StewardCONNECT, is now active. The portal allows you to connect securely with your doctor and medical team online, and to view and verify your health information. Log In or Activate an Account at www.stewardconnect.org. If you have any questions or concerns, email StewardCONNECT@Steward.org.

Where is health information stored?

All health data is stored on a secure server and managed with the protection of your information in mind. Only you – and those you authorize – will be able to see or gain access to the information in your Portal.

What to do after discharge from hospital?

After you’re discharged from the hospital, you can easily access discharge instructions and other information to help with your recovery. With proper authorization, you can manage a family member’s health records in his/her Portal account. Understanding and using your hospital Portal account will help you.

What is a representative account?

With proper authorization, a Representative account can be created that allows you to access and/or manage a selected family member’s health information just as with a personal account. Or, children may be registered as patients with the Portal, and authorized individuals may be granted access to their information.

How to access my health home?

When you sign-up for My Health Home, you will be able to: 1 View and share your personal health record (PHR) related to your hospital stays. 2 Have electronic access to an updated list of medications, diagnoses, allergies, lab test results, patient history and other healthcare information related to your inpatient hospital visit. 3 Access your hospital discharge instructions for better after-discharge care. 4 Manage family members’ health records once you are authorized.

What is my health home?

My Health Home patient portal is a free and secure online home for your inpatient hospital health information. The portal allows you to view your medical records on any internet-enabled device and become a more active partner in your healthcare.
