You may fax the completed form to 775-770-7368 or send US Mail to Saint Mary's Medical Group 411 West Sixth Street, Reno, NV 89503. Drop offs or pickups at this address are not available. Legal, insurance, and personal requests are subject to a fee. Invoices will be from Share Care. Please call 775-770-7626 with any questions.
