Accessing Your Patient Medical Records

Are you looking for copies of your medical records or test results? Carondelet Health Network is happy to help you obtain your medical records from our locations.

Process for Obtaining Records

To obtain your records, you or a representative must complete an “Authorization for Disclosure of Protected Health Information” form.

Records for Personal Use

If your medical record copies are for personal use, there may be a charge for photocopying a large number of records. If there is a charge, we will contact you with an estimated cost of completing your request for your medical records.

admitting & scheduling

Upon entering St. Joe's, a nonemergency patient will be admitted to the hospital at patient registration, which is located near the north entrance on the first floor. To be admitted, both medical history and personal information about the patient will be confidentially recorded for the use of the medical staff only.

what to bring

Patients should bring appropriate insurance forms or identification cards at the time of admission. Patients also should bring a few toiletry and personal items. Newborn babies will also need clothing and a car seat for their trip home. The medical center will provide all other necessary articles for your care.

Please note, we have visitor restrictions and screenings in place due to COVID-19 precautions, including limiting visitors to two per patient per day. Learn more.

Parking is free at all our locations. You are welcome to park in any of the designated lots.

St. Joseph is a weapons free campus. No weapons of any kind are allowed in the Medical Center.
