All documents and information relating to medical and mental health care, immunizations, and pharmacy needs are housed within the patient portal. You can use your St. John Fisher College network credentials to access the Patient Portal to enter your information and fill out forms as necessary. The forms below are now available on the portal.

Requirements for All Students

The College will require all students to be fully vaccinated against COVID-19 by August 1, 2021. View the Mandatory Student COVID-19 Vaccination Policy.

Uploading Immunization Information to the Patient Portal

All documents and information relating to medical and mental health care, immunizations, and pharmacy needs are housed within the Patient Portal. Students use their St. John Fisher College network credentials to log in to the Patient Portal to enter information and upload documents and forms as necessary.

Printing Your Immunization Record

For students and alumni enrolled after fall 2015, print your immunization records through the Patient Portal.

Requirements For All Students

• COVID-19 Vaccination Policy
  1. The College requires all students to be fully vaccinated against COVID-19. View the Mandatory Student COVID-19 Vaccination Policy and process for requesting an exemption.
• MMR
  1. If you are a student who was born after January 1, 1957 and are enrolled in six credit hours or more, you must provide the College with proof of immunity to measles, mumps, and rubella.
