How to get a copy of medical records?

To request a copy of your medical records: Complete an authorization to release information. Show proof of identity (Photo ID with name) General Requests. Hours: 9:00am - 4:00pm Monday - Friday, closed for lunch from 12:00pm - 12:30pm. Location: 4th Floor, North Wing.
