Request Your Medical Records

In the event that you need a complete copy of your SSM Health medical records, you'll need to complete and return the Authorization for Release of Protected Patient Health Information form to your local entity's Health Information Management (HIM) Department.

Include Proof of Identification

SSM Health takes patient privacy very seriously. Therefore, a copy of the patient’s legal guardian's ID is required to request medical records. We need this identification to verify that you are authorized to request the patient’s records. This can be a state-issued ID, birth certificate, etc.

Can you have one support person in a hospital?

Patients coming to the hospital for imaging studies or other diagnostic testing will be allowed one support person. This also applies to behavioral health patients and clinic patients. Exceptions may be made for end-of-life situations.

How many support people can a pediatric patient have?

All non-COVID positive patients, including pediatric patients, are welcome to have two support people per day, over the age of 16, during designated hours. One support person may stay overnight. All COVID-positive patients are allowed one support person with nursing leadership approval.

How many support people are allowed in an emergency room?

Emergency Room patients will be allowed two support people. The support people should remain the same for the duration of the visit. Interventional patients will be allowed two support people for the duration of their procedural encounter.
