With the patient portal, you can view select test results, access your medical records and track your health information. To enroll in the patient portal, you must receive services at one of our locations.
This information can include: your IP address, browser type, domain names, access times, and referring website addresses. This information is used by Family Care Network for the operation of the portal service, to maintain quality of the service, and to provide general statistics regarding use of the Family Care Network's patient portal website.

Family Care Network collects and uses your personal information to operate the Family Care Network patient portal, to validate and track communication and access of your personal health information, for future health information exchanges which we will notify our patients regarding, for access to certain health care data, as well as providing access regarding billing and payment information . We may also use this information and the portal to contact you with appointment reminders, health care preventive screening recommendations, surveys, or a new service that may be offered.
A family account allows multiple accounts to be linked together so they can be more easily managed together, by the patient (s).

To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
All messages sent to your provider’s office are transferred via a secure server compliant with HIPAA standards.
At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records. Due to the sensitive nature of radiology and pathology test results, these are not available on the patient portal and must be obtained through the hospital’s Health Information Department.
RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.

Legal guardians may request access to their ward's health information. Husbands and wives may request access to their spouse's health information. Adults may request access to their elderly parent's health information — with the patient's permission of course. Request Proxy Access.

The patient portal for Concord Hospital - Laconia and Concord Hospital - Franklin is separate from My Patient Connect. In the future, the two portals will be combined but for now, patients need to log in to separate portals based on where they receive their care.

Established in 1983 by Dr. Richard Michal, Rocky Mount Family Medical Center has grown from one provider and one nurse to eleven board-certified providers and a support staff of over fifty-five. Originally named Rocky Mount Family Medical Center, the name was changed to Family Medical Center of Rocky Mount in 2004 to allow for future expansion. In April 2002, Family Medical Center moved from Noel Lane to a new spacious facility at 804 English Road. With this move, new radiology and laboratory departments were opened to better accommodate our patients’ needs. Additionally in 2001, Family Medical Center began its movement toward offering the latest in medical technology with the addition of an electronic medical records system.

Medical conditions or problems you have discussed with your provider may not appear online; however, they are documented in your chart.
