Patient Portal allows patients to communicate with their doctors and access important information over the internet. Through the Patient Portal, your provider and clinic can send reminders, statements, lab results and more electronically. The Patient Portal also has a self-service payment option so that you can pay your bill online.

A portal can be established for any age, however, for patients between the ages of 12-18, a proxy agreement letter must be signed, and a proxy assigned if a parent/guardian would like access to the portal. Please email portal@swhealth.org for more information.
If you did not provide an email address during a past visit, you can contact us a portal@swhealth.org and request to be registered for the portal (s). Please be sure to specify in your email whether you want access to the Hospital Portal, Clinic Portal, or both.
An email address is required to create an account for either patient portal. For the hospital portal, multiple family members can use the same email address, however the clinic portal requires a separate email address for each user. If you do not have one, you may create an email account free of charge from various sources online.
Tests related to drug abuse. Pathology tests (including PAP tests, biopsies) are released to FollowMyHealth based on your doctor’s orders. Radiology results (including X-rays, ultrasounds, CT scans, MRI scans and mammograms) are released to FollowMyHealth based on your doctor’s orders.
When you do, that person becomes your health care proxy. “Read only access” means your health care proxy can only view your information.
If you use one of the sites listed above to sign in, FollowMy Health will not store your username and password. Your private information will only be stored by those sites.
