The brand new $50 million Cook Medical Plaza is located right off exit 37 on I-75. Facilities include a hospital with state-of-the art operating rooms, a 95-bed skilled nursing center, and a medical office building for physician clinics. The new Cook Medical Plaza opened Fall 2019.
The West Campus, conveniently located right off exit 64 on I-75, is home to both the Southwell Medical Clinic and the brand new $22 million Musculoskeletal Center that opened in 2018. The West Campus has over 45 physicians serving in a wide variety of specialty clinics.

Once Medical Records has set up your account, you can access your e-mail for a one-time user name, password, and link to the Portal from Southwest Medical Center Patient Portal. You can click on the link in your e-mail to enter this information , then set up your new user name and password. Returning users can access their account through the log in button below. Yes. Patient Portal passwords are encrypted and URLs are re-written so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your Patient Portal. Also, a timeout feature protects your information if you leave the Patient Portal page open.
Can my family access my Patient Portal? Yes, you can give family members, such as parents or healthcare proxies, access to your Patient Portal. This needs to be done within your healthcare facility and requires consent from you and your family member (s).
No, any e-vists that were in My Health Online will not show up in the new FollowMyHealth patient portal. New e-visits will need to be established in FollowMyHealth.
