What happened to the Boulder Community Health Patient Portal?

This patient portal is no longer being used by Boulder Community Health. Please call the practice to get information on how you can join their new portal. What is the community hospital patient portal?

This interactive web portal empowers all Community Hospital patients and their families to take a more active role in their care by providing easy, secure access to health information and online communication with Patient Portal staff. All of the information in the Patient Portal comes from your Community Hospital Electronic Health Record.

Is there a patient portal for CHP users?

This patient portal is not a wellness portal for our CHP users. If you are a School District 51, Mesa County or Strive employee, please visit the employer groups tab above. Securely message your health care provider For questions regarding the Patient Portal, please call (970) 644-4002.

What information will be available through the new mybch patient portal?

The majority of information available through the current portal will be available to you through the new MyBCH patient portal. However, some elements of your records, such as the complete history of visits, messages to your care team and notes from your clinic visits, will not be available.

How to log into MyBCH?

For the online portal, log into MyBCH, hover your cursor over the Profile icon in the top menu and select Personal Information. For the app, log into MyBCH click the three dots on the top right corner of the screen and select Account Settings.

How to obtain medical records from BCH?

Your complete medical records can always be obtained by contacting BCH Health Information Management at 303-415-7760 .

What can you do with an upcoming appointment?

If an upcoming appointment is eligible for eCheck-in, you can use it to take care of tasks such as checking in ahead of your appointment, updating personal, medical and insurance info, eSigning documents and answering questionnaires.

Hospital-related services, call Patient Financial Services at 303-415-4700. BCH physician bills, call CMA Physicians of BCH at 303-415-4766. For questions relating to a date of service on or after October 1, 2019: Please call the Patient Service Center at 303-415-5300.

How long does a BCH activation code last?

You can schedule a video or phone visit by calling your clinic directly , calling the BCH scheduling number at (303) 415-4015 or by requesting an appointment through your MyBCH portal. How old do you have to be to get medical records in Colorado?

To comply with Federal and Colorado State Laws regarding privacy and minors, Community Hospital has chosen to limit access to patient portals for minors over 12 years of age. Medical records for these minors can be obtained by contacting your provider’s office.

How old do you have to be to access MyHealth Portal?

Parents/guardians can request access to their minor child’s MyHealth Portal Portal, for children under 12 years of age, by clicking on the “I do not have an account” button above. Please provide the information for the child and sign as the parent/guardian.
