These interactive web portals empower all of our patients and their families to take a more active role in their care by providing easy, secure access to health information. All of the information contained in these portals comes from your Electronic Health Record.

We create a record of the care and services that patients receive from us. We need this record to provide patients with quality care and to comply with certain legal requirements. This notice applies to all records of patient care generated by Southeastern Medical Oncology Center, whether made by a physician or by others working in this practice.

Southeastern Health offers secure viewing and communication as a service to patients who wish to view parts of their records and communicate with our staff and physicians. Secure messaging can be a valuable communications tool, but has certain risks. In order to manage these risks we need to impose some conditions of participation.

About Us

We are a practice that couples compassion and dedication with experience and research in order to diagnose and treat cancer and blood related diseases. Our well-trained staff, which includes specially trained chemotherapy nurses, is dedicated to working with patients and their families to assure high quality treatment, therapy, and care.

How many locations does Southeastern Medical Oncology Center have?

This notice describes the privacy practices of Southeastern Medical Oncology Center, which at the current time has four locations. All locations will follow the terms of this notice and may share health information with each other for treatment, payment, or health care operations purposes described in this notice.

Is health information personal?

We understand that health information about patients is personal. We are committed to protecting the security of that information, called protected health information (PHI), and to preventing its disclosure without your authorization.

Can you revoke your health information?

If you provide us permission to use or disclose health information about you, you may revoke that permission, in writing, at any time. If you revoke your permission, we will no longer use or disclose protected health information (PHI) about you for the reasons covered by your written authorization. We cannot revoke any disclosures that we have already made with your permission. We are required to retain our records of the care that we provided to you.

How does a secure patient portal work?

A secure web portal is a kind of webpage that uses encryption to keep unauthorized persons from reading communications, information, or attachments. Secure messages and information can only be read by someone who knows the right password or pass- phrase to log in to the portal site.

Who must sign the authorization form?

Authorization/Signature. Authorization by the minor's parent or legal guardian is required. The minor's parent or legal guardian must sign this Authorization form. For minors 15 years old and older, the minor must also provide authorization and sign this form.

Can a parent/legal guardian dispute be resolved?

Access disputes between parent/legal guardian and minor, or between parents, cannot be resolved. If proxy access of a parent/legal guardian is revoked, the minor patient's Southeastern Health MyChart account will be suspended/terminated (a minor may not have an independent account; parent/legal guardian is required).

Who is Dr. Asaad?

Asaad is the newest member of the SMOC team. She is board-certified in oncology and completed her hematology/oncology fellowship at Penn State University – Milton S. Hershey Medical Center, Hershey, Pennsylvania. Dr. Asaad received her medical degree from the West Virginia University, Morgantown, West Virginia. She has a Bachelor’s degree in chemistry and biology from Bridgewater College, Bridgewater, Virginia.
