You can email the form and a picture of your ID to PatientConnect@southlakeregional.org. Already signed up? Provide your email address when you register as a patient at the hospital. Click here to enrol. An email will be sent to the same address that you entered on the Enrollment Request form.
Our online patient portal allows you to communicate with us easily and safely- on your schedule. Using your own secure password, you can log into the online patient portal 24 hours a day, 7 days a week from the comfort and privacy of your home, office, or mobile device.
Once your child is registered, you will receive a welcome email from South Lake Pediatrics. It will contain your temporary password and instructions for logging into your new, secure Patient Portal for the first time.
Patient Connect is a digital solution (web portal and mobile app) that provides patients and families 24/7 access to their healthcare information from Southlake Regional Health Centre, Markham-Stouffville Hospital and or Stevenson Memorial Hospital (s). If you are having problems enrolling online with Patient Connect, please call 1-877-733-5033.
Sign into your Patient Connect account. Click on the Profile Icon. Click on Update My Profile. Enter the information (street address, phone number) you wish changed, and press submit. Health Information will review your request, so the change may not appear immediately.
Patient Connect will automatically logoff after 15 minutes of inactivity.

Parents are able to view lab results, visit history, upcoming appointments, diagnostic and other reports.
If for some reason your health care provider disconnects from the virtual visit, please wait for them to re-join. Do not disconnect until provider has informed you that the visit is complete.
Parents with a proxy account access to their child’s health record will receive regular reminder notices that access to their child’s Patient Connect account will be automatically suspended when the child turns 16 years of age.
We will respond to appointment requests within the same business day if received by 4:30pm CST. Please call our scheduling line if you wish to schedule a same day appointment or use the healow app.
The Patient Portal is not intended for use in emergencies. If you require urgent or immediate medical care, call 911. Please call the clinic to schedule your child's first appointment. You may ask a receptionist to web-enable your account when you check in for your child's appointment.
