Once you provide us with your email address, our staff will enable your account. You will recieve a "welcome" email with log in instructions and your temporary username and password. Follow the link to Patient Portal, or visit our website and click the Patient Portal link on the home page to be routed to the Portal log in screen.

When arriving to your appointment you will be asked to present your insurance card and photo identification at check-in. Our staff will scan your cards and Id to ensure the correct information is on file. Insurance coverage will be verified upon each visit.
Payment for services are collected at check in. Co-payments and deductibles will be collected on the day of service. For your convenience we accept Visa, Mastercard, American Express, Cash or Check.
We are happy to accept new patients to our practice. After calling to set up your appointment you may print the New Patient Forms, fill them out ensuring past medical history is completed thoroughly and bring them in to your visit.
We ask that you call us 24 hours in advance to cancel your appointment to avoid a $25 cancellation fee.
Please allow our office 72 hours to process referrals or prior-authorizations required by your insurance company. Patients should contact their insurance company to verify if a referral is necessary for specialist visits. Please inform our staff if you will need a referral for an upcoming appointment.
Medications are refilled within 72 hours of your request. Please call our office 1 week in advance of running out of your medication. If you have not been seen in our office within the last six months of your refill we will require that you make an appointment and see the doctor.
