Getting help paying your bill

This is for anyone who receives services billed by Sky Lakes Medical Center. Other independent physicians, including anesthesiologists, surgeons, pathologists and other specialists may bill separately. Hospital Financial Assistance would not be applied to those bills.

How do I qualify for help?

You can ask for help with your bill at any time during your hospital stay or billing process. We will figure out how much you owe by looking at things like your income, assets, or other resources. If your yearly income is less than or equal to 250% of the current Federal Poverty Guideline (PDF), you may not have to pay your bill.

How can I apply for help?

You can apply for help with your bill in person, by mail, or over the phone by calling our Financial Assistance Department at 541.274.6699 between 8 a.m. and 4:30 p.m. weekdays. Financial Assistance Application (PDF).

Paperwork

You are responsible for taking part in our program to help you with your bill. In a timely way, you will need to share all of your information about your health benefits, income, assets, and any other paperwork that will help to see if you qualify. Paperwork might include income tax forms, check stubs, or other information.

Collection activities

We will make every effort to determine whether or not you are eligible for any sort of financial assistance during the first 120 days following your services. If during this time you do submit a Financial Assistance application, a 120-day extension may be applied to your accounts while a determination is made.

Our office is a family practice clinic, which also serves as a family medicine teaching center. Resident physicians provide patient care under the supervision of Oregon Health & Science University faculty physicians.

ACCESS TO ALL

Cascades East Family Medicine Center is committed to serving the community and its health needs. No one will be denied access to services due to the inability to pay.

MEDICATION REFILL POLICY

We encourage our patients to arrive 15 minutes prior to the start of their appointment and bring all of their medications. Patients should contact their pharmacy for refill requests. The pharmacy will contact Cascades East Family Medicine Center if there are no refills remaining.

PHONE CALLS

If you need to reach your doctor, simply call the clinic number (541) 274-6733. The front desk will forward a message to your health care team who will respond to your call within 48 hours. It is helpful to include alternate telephone numbers and best times to reach you as part of your message.

EPIC MYCHART

Cascades East encourages our patients to sign up for MyChart, which is a secure patient portal. Using MyChart, patients can message their health care team with questions, view their medical record and manage their account. To register for MyChart just ask the front desk staff when you are checking into or out of your appointment.

PATIENT ADVISORY COUNCIL

This council is made up of community members and providers and meets quarterly. Cascades East is proud to partner with our Patient and Family advisory council to improve the patient experience. If interested please inquire at the front desk.
