How do I register to the JCH Medical Group Portal?

Current Patients of any of the JCH Medical Group clinics: If you are currently a patient at any of JCH Medical Group, please contact a receptionist to assist you in generating a "JCH Medical Group Portal PIN." Once you have received your pin, click register and follow the instructions.

How do I sign in to the patient portal?

Select the Patient Portal icon shown on the right and follow the instructions appearing on the “Sign-in” page. For assistance with this feature, please contact Release of Information in the Health Information Management Department at (209) 468-6732, Monday – Friday, 8:30 am to 4:30 pm (Excluding Major Holidays).

Family Management allows all members of a family to be managed on one account. To add a member of your family to your Access Pass please click the Family Management link and send the JCH Medical Group the name and date of birth for each family member you wish to add to your Access Pass.
