The request must include the patient’s signature and date of birth. You can mail, email or fax the request directly to our Health Information Management (HIM) department. You may also drop it off at any PacMed clinic and ask our staff to interoffice your request to HIM.

In 2017, patients saw the first change in MyChart as PacMed joined the Swedish electronic health records (EHR) system, known as Epic. This got us on the same page, enabling us to join the Providence Epic now in June of 2019. How do I know my personal information is secure through the transition?
The portal is completely secure, so you can be confident that your private information is protected. Only you—or an authorized family member or caregiver—can access your information. I f you would like to assign a family member or someone you have a close relationship with, access (proxy) to your My SummitCare account, complete the My SummitCare Proxy Access Request Form below and return it to the medical records kiosk on the first floor of Summit Pacific’s Wellness Center.
Obtain an access code in person or by phone. You can ask at any PacMed clinic front desk for a printed access code and instructions for creating an account. You can also call the MyChart Helpdesk at 1.833.395.2035 to request an activation code, and an email will be sent to you with that information.
In June 2019 , PacMed completed an upgrade to its Epic electronic medical record and MyChart online tools, moving to the improved version used by our parent company, Providence St. Joseph Health, in Washington and Montana. If you are a MyChart user, you’ll see a few changes next time you log in.
If you do not remember any of this information, or you do not have a valid e-mail address on file, please use the password reset links within MyChart, or call our MyChart Helpdesk at 1.833.395.2035, Monday through Friday, 8 AM to 4:30 PM, and we will help you regain access to your MyChart account.

My Care (Patient Portal) is a website that allows you a convenient and secure way to view portions of your medical records and communicate with your provider’s health care team. Within your My Portal (Patient Portal) account, you will be able to do the following:
At your next visit, tell the staff you would like access to My Portal (Patient Portal). The staff will request your email address, and may have you complete a consent form. Within 30 minutes, you will receive an email invitation with a link to enroll. Just click on the link and follow the instructions to complete your enrollment.
Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a My Portal (Patient Portal) account with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age.
Some of the information in your My Portal (Patient Portal) account is provided through your medical health record and is viewable by you and authorized staff of Dignity Health facilities. If you would like someone to have access to your records to help manage your care, you can sign a release to authorize a trusted relative to be set up as a proxy.
The enrollment link expires 90 days from the day they are issued. If you are not able to find your invitation or it expired, you may request a new invitation from the staff at the facility that assist with your medical records.
