(A-G): (501) 743-1509 Medical Records has 30 days to complete each request. When requesting records we go back to 2012, the beginning of our electronic files. If you need beyond 2012 you must specify on your records request. You may fax records request to (501) 743-1550 or email them to medicalrecords@cabotmed.com.
Any prescriptions or samples should be requested 3 to 4 days before you run out. Once you have requested your prescription, it will be refilled within 72 hours and be sent to your regular pharmacy. If the prescription is controlled or requested by the patient, we will put a paper prescription at our front desk that must be signed out by the patient or someone listed on the most recent HIPAA form. Please check with your pharmacy before calling the clinic to check for your prescription.
Please allow at least a week to hear back from the referral clerk. Sometimes it takes a few days to get authorization from your insurance and/or hear back from the specialist.
Missed appointments without notification could be reason to dismiss you from the clinic. Please call 2 to 3 weeks in advance for Well-Child Check Ups, physicals, and appointments for medication refills. Patients can not schedule multiple appointments on the same day for insurance reasons.
If you request a copy of the information, we may charge a fee of $0.25 per page .
You have the right to inspect and obtain either a paper or electronic copy of medical information that is used to make decisions about your care. Usually, this includes medical and billing records, but does not include psychotherapy notes.
All phone messages left before 3:30pm will be read and addressed on the same day you call, if your provider/nurse is in the clinic that day. Please allow until the end of the day to get a call back from a nurse. If not medical emergencies, phone calls after 3:30pm may be addressed the next day.
We may disclose medical information about you to doctors, nurses, technicians, or other medical personnel who are involved in taking care of you at the hospital. For example, a doctor treating you for a broken leg may need to know if you have diabetes because diabetes may slow the healing process.
