Important Information for Parents

Shawnee Health Care has a legal responsibility to protect the privacy of a minor (age 12-17) who requests certain services. Illinois law allows a minor to confidentially seek care for services such as: birth control, sexually transmitted diseases, HIV testing, substance abuse, and mental health services.

Refills and Prescriptions

To request a prescription refill, please contact your pharmacy directly. If you don’t have any refills remaining, the pharmacy will send a request for a new prescription to your Shawnee Health Care provider.

Referrals

Shawnee Health Service is your first stop for health check-ups and wellness advice. Regular visits with your primary care doctor, nurse practitioner, or physician assistant can help you stay healthy. When you need the attention of a specialist, we will help with referrals to qualified local and regional medical and allied health professionals.

Appointments

Appointments are preferred though walk-in appointments are available on a limited basis.
