Health information may include blood test results, radiology results, your medical diagnoses, and more.

When preparing for your visit with LSFP, for the first time or the 100th time, you can help expedite a thorough and complete visit by making sure we have all the information necessary for treatment, including lab results, immunization records or specialist care notes. Visit our online health portal to ensure your records and forms are complete and up to date.
Please bring prior medical records, immunization records, all medication bottles (including vitamins and supplements) and ID verification that includes your current address as well as your insurance cards. If you are transferring your medical records from another practice, please call our office so we can establish your LSFP electronic record.
LSFP is there when you need us with same-day and urgent care appointments six-days a week, plus electronic records so you can be seen at any of our six convenient locations.
Payment is expected at the time services are rendered. We accept cash, personal checks, and most major credit cards. Patients are responsible for co-payments and any additional charges not covered by the carrier.
LSFP will process insurance forms for you. We do not, however, assume responsibility for collecting insurance payments or for negotiating settlements for disputed claims.
